Please use this form to order your new Standard Shades. All of the pricing is contained in this form except for repairs. You use the price chart included to view the price of each shade. Use one of the following methods for ordering:

1) Fill in the form below and submit it. You will receive a final quote including shipping and handling via email.

2) Fill in the form below. Print a copy of the order form below and fax to 724-763-3601.You will receive a final quote via email.

3) For Repairs - (Re-Cording only) - Send us email with shade type and dimensions for a price quote.

4) Payment must be made in advance. We will accept Master Card or Visa.
    We will also accept a personal check or money order made payable to OEM Shades Inc..

Freight  and Handling All orders will be shipped UPS.
  The first shade will be $8.85 and each additional
shade will be $5.00. There will be an oversize charge for shades 59" and over.
PA Residents Add 6% Sales Tax.
