2) Fill in the form below. Print a copy of the order form below and fax to 724-763-3601.You will receive a final quote via email.

3) For Repairs - (Re-Cording only) - Send us email with shade type and dimensions for a price quote.

4) Payment must be made in advance. We will accept Master Card or Visa.
We will also accept a personal check or money order made payable to OEM Shades Inc..

Freight and Handling	All orders will be shipped UPS. The first shade will be $11.55 and each additional shade will be $6.05. There will be an oversize charge for shades 59" and over.
